Complete the secure questionnaire on the following page. IMPORTANT: Bring your photo ID and primary/secondary insurance cards to your appointment to ensure your claim is processed properly. Alternatively if you prefer once you Book an Appointment go to www.rmc.md again, Click on patient forms and select Upload Patient Information and then you can upload a copy of your Insurance Card (front & back), Drivers License and Physicians Order if applicable Click here for the partial list of Health Insurance Plans we accept.

Your appointment is not verified until you receive a text or automated call containing a verification code and have entered that code on your online form. Be sure to keep your appointment form open in your browser until you have entered this code. If you do not immediately receive a text or automated call or have any questions, call 678-221-8000.

Please note that online appointments are for non-emergency illnesses or injuries. Unfortunately due to the requirements of Insurance companies, we cannot perform any test without the approval from your health insurance carrier, unless you are paying out of pocket.
